Intent to Return Form

This form is for use by Division of General Studies (DGS) students who stopped out at the University of Illinois voluntarily. In order to complete this form, you must meet all of the following conditions:

•    You withdrew during the semester or “stopped out” for a period of at least two semesters

•    You left Illinois either in good standing or on academic probation in DGS (Illinois GPA less than a
      2.0)

•    You have completed no more than five semesters of full-time coursework at Illinois and other higher       education institutions

The Intent to Return form may be either downloaded (requires Adobe Reader) or is available at the Campus Center for Advising and Academic Services. The form must be received according to the deadlines below:
